Remote Patient Monitoring platform
The m.Care platform is a remote patient, member or employee monitoring platform that is designed to monitor individuals at home that are dealing with medical, behavioral or mental health issues. Our clients will use the Care Management Portal to remotely monitor individuals who are using the m.Care mobile app. The
intent is to improve overall outcomes by showing, on a “real-time” basis,
whether an individual is improving or regressing after they are outside of the
client's direct care. The
platform uses data analytics and predictive modeling to proactively manage a
targeted population.
The Patient Engagement Mobile App has over 70 separate, yet
integrated, software applets that run on smart phones and tablets. Applications include tracking for physical
activity, nutrition, medication adherence, and monitoring of key vital signs.
Additional applets also securely capture videos and photographs, provide
education, gamification, and much more. m.Care integrates with fitness and Bluetooth-enabled devices and most
applets support voice recognition, providing significant flexibility and
utilization for the end user.
The
back-end system is an extensive, web-based, Care Management Portal, complete
with an easily configurable alert and task management system, as well as
dashboards for monitoring an assigned patient population. Clients can remotely assess an individual using the integrated Apple ResearchKit. Video recordings can capture an individual performing a remote assessment, including look detection, facial features recognition, and image heat mapping.
In summary, m.Care is a digital health platform for remote
patient assessment, engagement and monitoring.
Potential clients would be health systems, hospitals, pediatric hospitals, behavioral health and addiction treatment centers, and providers of corporate wellness services.
